I've noticed a couple times that when I flag an answer on a question, mainly because it's the relevant piece of the flag, the question title for that flag doesn't get updated on the flagging review page. Namely, often times I'll flag a "not a real answer" with an extra message, in this case that the question is probably too localized. The question was then closed as too localized and the flag marked as helpful, but the flagging page doesn't update the title with the [closed] part. Thus, it looks like a moderator has dismissed the flag as helpful since the title doesn't say closed and it wasn't deleted (no red background).

@Cody: If you flag an answer, and the question gets closed, the title for that flag never gets updated with [closed]. –  animuson Feb 26 '12 at 5:14
