I've just updated my Reputation Tracker.

It now really does cover SO, SF, SU and Meta [and Programmers SE -ed.].

Additionally, it has the option of including your flair for each site to display current reputation and badge counts.

The IDs for the different sites are stored in the so, sf, su and meta parameters. For example, my bookmark is (formatted for reading convenience — obviously join the two lines together!):

Old bookmarks (with user=xxx) will show rep for Stack Overflow only (even if you've got sites=meta) but the ID will be in the text box, ready for you to resubmit and bookmark.

You should get SO to put a link to the reputation tracker on a Users Profile page, so it only takes a button push.
