The minimum reputation is 1. You just need to have an account and be logged in for the redirect not to be applied; it only applies to anonymous (logged-out) users:
Starting with the next build, anonymous users will be redirected when they visit duplicate questions with no answers.
As with migration and merge redirects you can disable with ?noredirect=...

Can't find an official post on this, but the highest amount of dupe-targets I've encountered is five. I wrote a quick SEDE query sorting the posts by number of dupe targets coming from it, and this seems to confirm that "five" assumption.

Consequently, we want every one of those questions to be unique and to pose a clear question so that it attracts one or more clear answers.

This has been fixed.
There had been one part of the code that was getting the duplicate selections, and another where it was only showing the duplicate selections if they had an answer. So if there was only one suggestion and it didn't have an answer, you would see the notice but would not be given anything to choose from.
